Submitted forms with incomplete or inaccurate information will not be processed. Exam or desk copy requests can be typed on college letterhead and submitted; however all the information requested on the PDF request form must be included. Please mail or fax the completed form to:

Exam (Review) Copy Request Form:
Instructors can request one complimentary book for examination/review prior to ordering quantities for use as a course textbook. If a minimum of 10 copies of the book are not purchased within 45 days, and the complimentary exam textbook is not returned in salable condition, the instructor may be billed the full price of the textbook.

The instructor may keep the complimentary exam book for use as a desk copy if the minimum number of textbooks is purchased. The instructor is not eligible to receive an additional complimentary desk copy.

Exam (Review) Copy Request Form (PDF)

Desk Copy Request:

Instructors can request one complimentary desk copy of a title if a minimum of 10 copies of the textbook are purchased. If an instructor has received a complimentary exam copy of the textbook, the request for an additional desk copy will not be fulfilled. In situations where multiple instructors teach a single class, each instructor must register individually to receive a desk copy and a minimum of 10 student copies must be purchased for each five desk copies. Insturctors may be billed for the full price of the textbook if book orders are not placed.
